Here is another alternative to the Trellix/CSB built-in form Mailer Wizard. The web site CGISpy.com has many different CGI scripts to choose from, and will remotely host your form/emailer app. Best of all, it's free!

How to use:

Preliminary:

1. Go to CGISpy.com and sign up (it's free). After that, sign in with your username and password to go to the Administrative area.

2. Click on the 'Email Form' link

3. After entering your desired information, click on the button to get your email code.

4. The code provided can be copied into an 'Insert HTML'
